Appointments Our physicians work by appointments which can be made by
phone or in person. You may request the doctor of your choice or the first available.
While every effort is made to ensure that you are seen at your scheduled time,
all appointment times are approximate as individual patients with the same problem
require different amounts of time. Also, emergencies do occur and surgeries
can run longer than expected; thus, delays are often unavoidable. Your patience
is appreciated.
Your arrival on time will help us stay on schedule. If we are running behind,
you may wait to see the doctor, or reschedule.
Cancellations
If you need to cancel or reschedule, please notify us as far in advance as possible.
This allows us time to accommodate other patients. Regrettably, emergencies
will occasionally require us to reschedule your appointment with little or no
notice.
Initial Visit Please
arrive early for this visit as you will be asked to complete a Patient
Information Form and also a Medical History Form.
The forms can be found under the Registration Forms section on our web site.
Records Release
Your medical records are held in strict confidence. Information can be released
to other physicians, insurance companies, attorneys, or other agencies only
after you have signed a release form.
Unscheduled Visits
If you have an acute injury or problem, we will work you into our schedule,
please call first. If your physician is not available, one of the other physicians
will be glad to see you.
Telephone Procedures
All telephone calls are received through our switchboard, then directed to the
appropriate person. When you call please state the nature of your call so that
it can be directed appropriately.
Emergency calls will be handled promptly. Your information will be taken by
a member of our staff and a return call made as soon as possible.
Our staff is trained to answer most routine questions, however if they are unable,
a physician or their assistant will return your call. Routine calls for the
physicians are handled at the end of office hours.
Prescription Refills
For refills please, contact your pharmacy directly. They will then contact our office. Please, don't call for refills outside of normal office hours.
After Hours
In case of an emergency, one of our physicians is available for advice or emergency
care. Our answering service will take your information and contact the doctor
on call. Routine matters should be addressed during regular working hours.
Billing & Payment
Unless prior arrangements are made, payment is expected at the time of the office
visit. You may request an itemized statement including all charges and your
diagnosis at the end of your visit.
As a courtesy to you, we will file your insurance claim for you, if you provide
us with the necessary information. Ultimately, you are the responsible party
for your bill.
A monthly statement will be sent out on all outstanding accounts. Accounts without
regular payment will be turned over to our collection agency.
Financial Hardship
Unfortunately, there are times when circumstances make it difficult to meet
your obligation. If you are in this situation, please discuss it with our bookkeepers.
We are always willing to work out payment arrangements with our patients, and
wish to avoid collection proceedings.
